Its an excellent idea to start a blog on MTEF. I am also working on MTEFs at the Sub-national level.Congratulations on being recognized by IMF_PFM.
In India MAI would mean mother, hope this becomes mother of PFM.

Dear MAI,
I think "From Line-item to Program Budgeting" edited by John M Kim is a good document on how MTEF is at the core of Program Budgeting.
I am also currently working in evovling MTEFs for selected departments for a DFID funded project in India.
